Registered Nurse | Integrated Family Health Service (IFHS)

About the Role
We are seeking a dedicated Registered Nurse to join our interprofessional Integrated Family Health Service (IFHS) team. This role focuses on providing holistic healthcare services to children, adults, and whānau, enhancing health outcomes in rural communities. Working within the primary care setting, you will align your efforts with Health NZ’s vision and values to deliver culturally competent, person-centered care.
Key Responsibilities
- Clinical Care Delivery
- Provide high-quality care in primary care settings for improved patient outcomes.
- Prioritize the needs of Māori as tangata whenua to achieve equitable health outcomes.
- Undertake comprehensive, culturally appropriate assessments and care planning.
- Utilize evidence-based practices for holistic health management.
- Interprofessional Collaboration
- Support the integration of acute, community, mental health, and primary care services.
- Collaborate with community stakeholders to address unmet healthcare needs.
- Promote wellness by empowering whānau through health literacy education.
- Continuous Improvement
- Embrace changes to health strategies and models of care with enthusiasm.
- Enhance personal professional development (e.g., Registered Nurse Prescribing, mental health training).
- Identify opportunities to streamline the client journey for equitable outcomes.
- Professional and Ethical Standards
- Adhere to professional, ethical, and legal standards in decision-making and conduct.
- Support a culturally safe environment and uphold the Treaty of Waitangi principles.
- Patient and Whānau Advocacy
- Advocate for patients and address barriers to accessing health services.
- Provide education tailored to improve health literacy and empower clients.
About the Team/Service/ Location
The Practice Nurse is a highly trained Registered Nurse and recognised member of the interprofessional Integrated Family Health Service (IFHS) team located in the Central Region (Greymouth). This role is responsible for providing a range of holistic services across the age continuum within the integrated primary care setting; enabling excellence in rural health outcomes in alignment with Health New Zealand- Te Tai o Poutini’s vision and values.
About You
Qualifications and Skills
Essential:
- New Zealand Registered Nurse with a current Annual Practising Certificate.
- Commitment to cultural competency training and application.
- Proficient in MedTech32 and Microsoft Office Suite.
Desirable:
- Experience as a Practice Nurse, including long-term condition management.
- Current certifications in immunization, cervical smears, and triage.
- Postgraduate qualifications in pathophysiology, pharmacology, or other relevant fields.
Attributes:
- Passionate about rural health and Māori health equity.
- Flexible, collaborative, and self-motivated team player.
- Resilient under pressure with excellent critical thinking and communication skills.
