About the Role

We are looking for compassionate and skilled Registered Nurses to join the Kahurangi Dementia Unit. In this rewarding role, you will use your nursing expertise and clinical judgement to assess health needs, deliver high-quality care, and support residents living with dementia to maintain their wellbeing, dignity, and independence.

Working collaboratively with an interdisciplinary team, you will provide comprehensive nursing assessments, develop and implement care plans, and deliver evidence-based nursing interventions. You will also guide and support Enrolled Nurses, Health Care Assistants, and other team members to ensure residents receive safe, effective, and person-centred care.

Key Responsibilities

Clinical Care & Care Planning

  • Undertake comprehensive nursing assessments and contribute to holistic care planning, including InterRAI assessments where appropriate.
  • Develop, implement, and evaluate individualised care plans in partnership with residents and their whānau.
  • Utilise evidence-based practice to guide clinical decision-making.
  • Provide education and support to residents and families to promote wellbeing and health literacy.

Coordination & Collaboration

  • Work collaboratively with the multidisciplinary team to ensure timely access to services and coordinated care.
  • Advocate for residents and support their involvement in decisions about their care.
  • Coordinate referrals and support transitions across services where required.
  • Promote effective communication within the team and across services.

Documentation & Quality

  • Maintain accurate and timely clinical documentation in accordance with organisational policies.
  • Ensure patient information is managed in line with the Health Information Privacy Code.
  • Contribute to quality improvement initiatives and safe practice environments.
  • Support restraint minimisation practices and patient safety initiatives.

Leadership & Team Support

  • Delegate and supervise care provided by Enrolled Nurses, Health Care Assistants and support staff.
  • Promote a positive team environment and support the development of colleagues.
  • Role model professional communication and teamwork.

About You

Essential

  • Registered Nurse with the Nursing Council of New Zealand and a current Annual Practising Certificate with no conditions affecting practice.
  • Participation in, or working towards, the Professional Development and Recognition Programme (PDRP).
  • Commitment to cultural competency and culturally safe practice.
  • Strong communication, clinical assessment, and organisational skills.
  • Computer literacy including Microsoft Office.
  • Full, clean New Zealand driver’s licence (manual driving ability may be required).
  • Ability to participate in on-call duties when required.

Desirable

  • Experience with InterRAI assessments.
  • Previous experience working in aged care or dementia services.
  • Demonstrated leadership and teamwork skills.
  • Strong clinical competence and organisational ability.
  • Motivation to contribute to the ongoing development of the unit.
